Assessment & Referral
Stop wondering if you have an addiction problem… We can help.
To better address your specific needs, an individual assessment may be necessary. Individuals can call our office to schedule an appointment for an assessment. The phone number to call is (314) 962-3456. Appointments can be scheduled for an assessment Monday through Friday from 9:30 am until 3:00 pm. The cost is fifty dollars and the assessment lasts for about an hour. ASSESSMENTS ARE DONE BY APPOINTMENT ONLY.
The Process of an Adult Assessment
There are three evaluation forms to be filled out by the individual, the Drug Abuse Screening Test, the Heilman, and the Mortimer-Filkins Questionnaire. All three forms are made up of signs and symptoms of chemical dependency taken from the Diagnostic and Statistical Manual of Mental Disorders - Fourth Edition. Once the forms are completed and scored, a verbal evaluation is performed with similar questions, but more detail is obtained.
When this process is completed a form letter is written stating the presence of the individual, the scores on the evaluations, whether they met criteria or not for chemical dependency and our recommendation for the individual. All assessments are performed by qualified information and referral specialists with many years of experience in the chemical dependency field. The fee for an adult assessment is $50.
Types of Recommendations for Adults
Recommendations can include one or more than one of the following activities:
- Attendance at educational/informational meetings on chemical dependency.
- Attendance at "Self Help" meetings.
- In-patient and/or out-patient chemical dependency treatment.
- Individual therapy with qualified therapist specializing in chemical dependency.
- There are occasions when no recommendation is made.
- If any additional issues are identified, referrals to other resources can be made.
The Process of an Adolescent Assessment
Parents can call our office to schedule an appointment for the adolescent. Due to the nature of the evaluation given to adolescents, this assessment process can take up to two hours. To allow enough time adolescent assessments can be scheduled from 9:30 am until 2:30 pm Monday through Friday. Assessments for youth ages 18 and under are free.
The adolescent assessment begins by administering an evaluation form to the teen and also to the
parent(s) or guardian. The assessment given to the teen is called the Problem Oriented Screening Instrument for Teenagers (POSIT) and the assessment for the parent is the Problem Oriented Screening Instrument for Parents (POSIP). The areas covered in these evaluations are: substance use/abuse; mental health; family relations; peer relations; and aggressive behavior/delinquency. Upon completion these forms are scored, followed by a verbal evaluation with the adolescent without the parent. The parent(s) is then asked for any additional information followed by a meeting with both the teen and the parent(s). Results are discussed and referrals are made at this time.
